Dear Subscriber, you will need to go to USAC first to apply for the Lifeline National Qualifier before proceeding to the Gearheart Broadband / Fiber Opt-in to enroll form
If you have already applied for the National Qualifier you may proceed to Gearheart Broadband / Fiber OPT-IN form submission
*You must be a Resident of the State of Kentucky to Qualify for Lifeline.
